To apply for the Written or Practical Examination please download the application form by clicking on the link below.
Methods of payment: Personal check, company check, USPS money order or Visa / Mastercard.
If paying by credit card, complete the form and fax to: NCSCBinc, "Attention Mike Iacobellis" to the fax number on the form.
If paying by check or money order, complete the form and mail to: NCSCBinc, "Attention Mike Iacobellis" at the address on the form.
Please make check payable to: NCSCBinc.
The form must be filled out completely and a signature on page two of the form.

Certification Fees
The fees for the certification are as follows:
$295.00 Note: Payment must accompany application.
$595.00. Note: Payment must accompany application.
These fees include all cable, tools for use during the exam* and cable accessories such as Separable connectors (elbows), splices and terminations.
*Excludes personal hand tools

Application and payment should be received at the NCSCBinc office twenty one (21) days prior to the examination date. If you would like to participate in a scheduled test that is inside the 21 day window please contact the NCSCBinc @ 480-882-9424.
Due to scheduling and the cost associated with administering the examinations, all candidates for the written examination who cancels his or her test twenty one (21) days or less from the examination date a penalty of $200.00 will be assessed. If a candidate for the Practical examination cancels his or her test twenty one (21) days or less from the examination date a penalty of $400.00 will be assessed.
If it becomes necessary for the NCSCBinc to cancel a test payment will be refunded in full.

If a candidate for the Practical examination cancels his or her test twenty one (21) days or less from the examination date a penalty of $400.00 will be assessed.
